Your assessment results and clinical information will not be shared with the payer or any outside provider unless you or your legal representative authorize the disclosure, or unless disclosure is otherwise permitted or required by law. You may request that your assessment report be sent to a provider of your choice.

Time to Evaluate does not bill Medicare, Medicaid, TRICARE, CHAMPVA, or any other federal health care program for assessment services. If you choose to seek reimbursement from an insurer, health plan, HSA/FSA administrator, or other source, you are responsible for confirming eligibility and requirements.
